My number one, based on experiences with companies, is that they go after a framework (to "do" Agile) before they understand what it will/would mean to "be" Agile. This usually means their exposure/training skips, or only very briefly discusses, the Agile Manifesto's Values and Principles. This means they believe "being" Agile is about "doing" the framework.
